Possible psychological and environmental factors
When men believe that they have no control over their ability to withhold their ejaculation time, psychological factors will often contribute to Premature Ejaculation. Men sometimes underestimate the relationship between sexual performance and emotional well-being. Premature Ejaculation can sometimes be caused by temporary depression, or other factors, such as, stress over financial matters. A very common cause, however, is that such men may have unrealistic expectations about their own sexual performance. For example, if a man believes he will fail to control his ejaculation, that only serves to worsten the problem, due to the overall lack of confidence that he will be successful in withholding ejaculation long enough. Interpersonal dynamics strongly contribute to sexual function. Premature Ejaculation tends to cause a lack of communication between partners, hurt feelings, or unresolved conflicts that further interfere with the ability to achieve emotional intimacy. In a less pathological context, sometimes Premature Ejaculation can also be simply caused by extreme arousal. In other words, men can become so exited that they cannot withhold ejaculation.
Mechanism of Ejaculation
The emission phase is the first phase. It involves some of the fluid escaping into the urethra within the penis. The second phase is the expulsion phase, which involves closure of the bladder neck, followed by the rhythmic contractions of the urethra by the pelvic muscles and the urethral sphincters. It is also believed that the neurotransmitter serotonin (5HT) plays a central role in the ejaculation process. It is believed that low level of serotonin in specific areas in the brain could contribute to the causes of Premature Ejaculation.
Motor neurons located in the thoracolumbar and lumbosacral spinal cord are activated in a coordinated manner when sufficient sensory input to reach the ejaculatory threshold has entered the central nervous system. Other researchers have noted that men who suffer from Premature Ejaculation have a faster neurological response in the pelvic muscles.
Several areas in the brain have also been identified to be involved in control over ejaculation. Scientists have long suspected a genetic link to certain forms of Premature Ejaculation. In one study, 91% of men who suffered from lifelong Premature Ejaculation also had a first-relative with Premature Ejaculation, which lasted their entire life.
Diagnosis
Diagnostic for Premature Ejaculation DSM-IV-TR (American Psychiatric Association) is:
A. Persistent or recurrent ejaculation with minimal sexual stimulation before, on, or shortly after penetration and before the person wishes it. The clinician must take into account factors that affect duration of the excitement phase, such as age, novelty of the sexual partner or situation, and recent frequency of sexual activity.
B. The disturbance causes marked distress or interpersonal difficulty.
C. The premature ejaculation is not due exclusively to the direct effects of a substance (e.g., withdrawal from opioids).
